Job Description:
UCHealth, a leading nonprofit healthcare system in Colorado, is
seeking an experienced and compassionate Care Manager RN to join
our team. This role is pivotal in managing patient care
coordination and ensuring that our diverse patient base receives
optimal health outcomes. By working closely with patients,
families, and other healthcare professionals, you will develop,
implement, and evaluate individualized patient care plans. This
position requires a deep understanding of healthcare regulations,
patient rights, and the ability to function effectively under
pressure within a fast-paced environment. Assess patient needs and
develop comprehensive care plans. Coordinate with healthcare
providers to ensure all aspects of care are addressed. Monitor
patient progress and adjust care plans as necessary. Educate
patients and families about health conditions and care maintenance.
Participate in multidisciplinary team meetings to review cases and
improve care delivery. Ensure compliance with healthcare standards,
laws, and regulations. Strong clinical skills with a focus on care
management and patient advocacy. Excellent communication and
interpersonal skills. Ability to work collaboratively in a
multidisciplinary health team. Highly organized with strong
problem-solving skills. Proficiency in electronic health record
(EHR) systems. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Valid Registered Nurse (RN) license.
Certification in Case Management (CCM) preferred but not
required.
